The tear trough is one specific groove, running from the inner corner of the eye down towards the cheek. Under-eye filler addresses hollowness across the wider area, which may include that groove and the junction between the lower lid and the cheek. Dr Selvi will advise which describes your concern. Could my cheeks be part of the cause? Sometimes. As the cheek loses volume, the under-eye area loses support and the hollow can deepen. Cheek filler can improve the look of under-eye hollows by restoring that support, and for some people it is the better first step. I have puffiness or dark skin rather than hollows. Will this help? Probably not. Puffiness, fluid retention, under-eye bags from fat pads, pigmentation and visible blood vessels under thin skin are not corrected by filler, and puffiness can be made worse. Treatments that work on the skin itself, such as polynucleotides or skin boosters, may be discussed instead. What are the risks? Swelling and bruising are common. The rare, serious risk is vascular occlusion, where filler blocks or presses on a blood vessel and reduces the blood supply to the skin. Close to the eye this can, very rarely, affect vision. Pale or blotchy skin or increasing pain means phoning the clinic straight away; any change in vision needs 999 or A&E first. Hyaluronidase is kept on site so treatment can start immediately. Is it reversible? Yes. Hyaluronic acid filler can be dissolved with hyaluronidase if there is a complication or if you are unhappy once the area has settled. Dissolving has its own considerations, so it is discussed with you rather than treated as a casual undo. How long will I look swollen? Swelling usually peaks at 24–48 hours, and the under-eye can take longer to settle than other areas. Any bruising fades over 7–14 days. Allow at least two weeks before an important event.
